Home health care brings skilled nursing, therapy, and related support into your home. Depending on your needs, care may help you recover, manage a condition, maintain function, or slow a decline.

We provide skilled nursing, physical therapy, occupational therapy, speech therapy, home health aide services, and medical social services. Your care team determines which services are appropriate based on your assessment and your healthcare provider’s orders.

You must be homebound, need qualifying skilled care, and have care ordered and certified by a physician or other authorized healthcare provider. Additional requirements apply. Our team reviews your referral and helps verify eligibility.

Because of an illness or injury, you need assistance, a mobility device, or special transportation to leave homeor leaving is medically inadvisable. You must also normally be unable to leave home, with outings requiring considerable effort.

For example, a patient recovering from surgery may need a walker and another persons help, while pain and weakness make leaving home exhausting. A walker or diagnosis alone does not establish eligibility.

Yes. Being homebound does not mean you must stay indoors at all times. Medical appointments, religious services, and short or infrequent outings do not automatically affect eligibility, provided you continue to meet the homebound requirements.

A healthcare provider must order your home health care. Referrals may come from a healthcare provider’s office, hospital, or rehabilitation facility. Patients and families can also contact our team for help coordinating a referral.

No. A hospital stay is not required for Medicare home health eligibility. You may qualify while living at home if you meet the applicable requirements.

Coverage depends on your plan, eligibility, and care needs. Our team verifies coverage, network participation, and any authorization requirements before admission. We explain any known patient financial responsibility before services begin.

Visits follow your healthcare providers orders and individualized plan of care. Your schedule depends on the services you need, your condition, and applicable coverage requirements. Your care team will explain the planned frequency and coordinate visits with you.

Home health aides may assist with bathing, grooming, dressing, and other assigned personal care tasks. Under Medicare, these services must accompany qualifying skilled care; personal care alone is not covered through the home health benefit.

Home health provides scheduled clinical visits and related support. It does not provide someone who stays in your home continuously. If you need ongoing supervision or daily caregiving, our team can discuss additional support resources.

Yes. Living alone or having a caregiver does not automatically prevent you from receiving home health care. Eligibility depends on your clinical needs and coverage requirements. Our team assesses safety and works with available caregivers when appropriate.

Improvement is not always required. Medicare may cover skilled care needed to maintain your condition or slow deterioration when all other requirements are met. Continued services depend on the need for skilled care and an individualized assessment.

A nurse or therapist reviews your health history, medications, symptoms, abilities, home safety, and care goals. They explain recommended services, review your rights and responsibilities, and coordinate your plan with your healthcare provider.

Please have your current medications, insurance information, healthcare provider contact details, and any recent discharge instructions available. If a family member or caregiver helps manage your care, having them present can be helpful.

Contact our office as soon as possible so we can coordinate with your clinician. Please let us know about appointments, hospital visits, or other changes that may affect your schedule or care.
