Homebound status: Can I still leave home?
Yes. Medical appointments, religious services, and short or infrequent outings do not automatically disqualify you. What matters is whether you continue to meet the homebound criteria.
Your Care Plan and Coverage
Each patient receives services based on medical necessity and an individualized plan. You may need one discipline or several working together.
Medicare home health provides intermittent skilled care. It does not cover continuous, around-the-clock home care or housekeeping unrelated to your care plan. Medicare Advantage and other insurance plans may have network and authorization requirements.
For Medicare, occupational therapy generally cannot establish initial eligibility by itself, but it may continue after eligibility has been established through another qualifying skilled service.

Skilled Nursing
Support for your health and recovery
Our nurses assess your condition, provide ordered treatments, and teach you and your caregivers how to manage your care between visits.
Nursing services may include:
- Health assessments: Monitoring symptoms, vital signs, and changes that need follow-up with your healthcare provider.
- Medication education: Reviewing your medication list and explaining prescribed use, precautions, and possible side effects.
- Chronic condition management: Education and monitoring for conditions such as diabetes, heart failure, and respiratory disease.
- Wound care: Assessing wounds, performing ordered dressing changes, and teaching skin protection and infection warning signs.
- Recovery after hospitalization or surgery: Reviewing discharge instructions and helping you understand your care needs.
- Caregiver training: Teaching safe care techniques and when to contact the office or seek medical attention.
Your nurse communicates relevant findings to your healthcare provider and helps coordinate changes to your care.
Rehabilitation Therapies
Care that supports mobility, independence, and daily function.
Physical Therapy
Build strength and move more safely. Our physical therapists evaluate how you move and create an individualized treatment plan to support mobility and function.
Services may include:
- Exercises to improve strength, flexibility, and endurance.
- Walking and balance training.
- Training for getting in and out of bed, chairs, and other surfaces.
- Instruction in using walkers, canes, and other mobility devices.
- Fall-risk assessment and home safety recommendations.
- Patient and caregiver education for safe movement.
Treatment may support recovery after surgery, stroke, injury, or a decline in mobility. When skilled therapy is necessary, care may also help maintain function or slow further decline.
Occupational Therapy
Make everyday activities easier and safer. Our occupational therapists help you adapt to physical or cognitive changes that affect daily living.
Services may include:
- Training for dressing, bathing, grooming, and toileting.
- Strategies for safely completing household activities.
- Upper-body coordination and functional exercises.
- Instruction in adaptive equipment.
- Energy conservation and activity pacing.
- Home safety recommendations.
- Strategies to support routines, memory, and task completion.
- Caregiver education.
The goal is to help you participate in meaningful daily activities with greater safety and independence.
Speech Therapy
Support communication, thinking skills, and swallowing. Our speech-language pathologists evaluate and treat difficulties that may affect communication, cognitive skills, or swallowing.
Services may include:
- Speech and language treatment following stroke or other illness.
- Strategies to improve understanding and expression.
- Memory, attention, and problem-solving exercises related to daily function.
- Swallowing evaluation and individualized treatment.
- Education on recommended swallowing precautions.
- Patient and caregiver training.
Your therapist coordinates recommendations with your healthcare provider and care team.
